What Is Round Ligament Pain?
The round ligaments are two thick, cord-like structures that run from the top of your uterus, down through the inguinal canal, and into the labia majora. Their whole job is to support the uterus and keep it in the right position inside your pelvis. Before pregnancy they are relatively short and taut. As your uterus grows, expanding from roughly the size of a pear to the size of a watermelon, those ligaments stretch significantly, sometimes doubling or tripling in length.
All that stretching is what causes round ligament pain. The ligaments are full of smooth muscle fibers and nerve endings, and when they get suddenly pulled taut, by a quick movement, a sneeze, a cough, rolling over, or even a good belly laugh, they can spasm. The result is that sharp, stabbing, or cramping sensation in your lower abdomen or groin, usually on one side but sometimes both. It typically lasts just a few seconds and then fades. Completely normal. But when it is happening multiple times a day? Not exactly fun.
Round ligament pain is most common between 14 and 27 weeks but plenty of moms feel it well into the third trimester. And while it is harmless, for some moms it is frequent enough to genuinely disrupt daily life, and that is where we come in.

Here is something that does not get talked about enough: round ligament pain is not just about how big your belly is getting. If it were purely about size, every mom would have the same experience. But they do not, and the reason comes down to pelvic alignment.
The round ligaments are part of a whole network of ligaments, muscles, and connective tissue that support the uterus and pelvis. When the sacrum and sacroiliac joints are subluxated, meaning misaligned or restricted in their movement, the entire pelvic structure becomes asymmetric. One round ligament ends up under significantly more tension than the other, which makes it way more prone to spasm. The piriformis muscle, the psoas, and the broad ligament all play into this too.
